Registering with us
When registering with the practice please bring your medical card. You will be given an appointment for a registration health check with the nurse.

It is essential you attend this appointment as it provides the practice with important information on your current health needs. New patients to the practice will be required to fill in a medical form which can be obtained from the reception.

GP Registrars

Please note we actively support medical training at this practice and as such we have GP Registrars (trainees) who are qualified doctors working under supervision.

WITH YOUR PRIOR PERMISSION the registrars will be video recording some of their consultations as part of their training.
We would appreciate and value your cooperation however, we will respect your decision not to get involved in the training process. If you do not want to take part then please inform either the reception or the doctor you are seeing.

APPOINTMENTS

All surgeries are by appointment only.

Appointments may be made by telephoning 020 8524 1777 or in person during surgery hours.

You may see any doctor at the practice regardless of which one you are registered with. Please report to the reception when you arrive for an appointment. If you are unable to attend your appointment please notify the practice in advance so that we can offer the appointment to someone else.

We make every effort to keep to appointment times, but difficult clinical situations may cause some delays. When there is likely to be long delays we will:

inform patients in the waiting area and

offer alternative appointments if required.

Urgent Appointments

If you have a problem that cannot wait for a routine appointment you will be offered an appointment at the end of morning surgery.

HOME VISITS

If you are requesting a home visit please try to telephone before 10.00am and leave a contact number, as the doctor will normally wish to speak to you before accepting a home visit request.

These visits are for those who are housebound or far too ill to come to the surgery.

REPEAT PRESCRIPTIONS

If agreed by the doctor repeat prescriptions can be supplied by returning the repeat prescription request form attached to your prescription.

Tick the items required and place your form in the prescription box at reception. Your medication review date will also be displayed on this form.

Please allow 48 hours before collecting your script.

Prescription requests will not be taken over the phone.

TEST SERVICES

We are able to organise a variety of tests on your behalf. Please read the following as it may be useful to you.

Diabetes

Are you at risk? If you are over 50 and have one of the following you may be at risk of having diabetes:

Obesity

First degree relative with diabetes

History of Gestantional diabetes or large babies

High blood pressure

Recurrent infections

Previous stroke or heart attack

If you would like to be tested then please speak with one of the practice nurses.

Fasting Blood Test For Diabetes

If you are having the above test then you must not have anything to eat or drink from midnight(except a small amount of water to take any tablets with).
You should then attend either:

Chingford Hospital (silverthorne centre) or

Whipps Cross Hospital

for a blood test that following morning.

After your your blood test you can eat and drink as normal.

If you have any queries then please speak to the practice nurse.

Cytology

Churchill Medical Centre offer cervical smear tests to all women aged 20 to 64. If you are over 65, or have had a hysterectomy or if you have any doubts about whether you still need to be tested please see one of the clinicians.

REGULAR SMEARS ARE IMPORTANT. THEY PICK UP THE EARLY WARNING SIGNALS THAT COULD SAVE YOUR LIFE

Test Results

First Non-clinical staff will not and do not give test results.

For results please allow:

ONE WEEK for all blood and urine tests carried out in the lab.

10 DAYS for X-Rays

If you wish to enquire about your test results, please call the practice after 11.30am.
Please do not call before this time as it will not be possible to help with your enquiry.

Sick Notes

For the first 5 days of sickness, you do not need a sick note.

If your employer requests a sick note for less than 5 days absence, then there is a standard fee for this service (in line with national BMA guidelines).

A standard sick note (after a weeks illness) is free of charge.

Letters, Reports & Forms

Certain requests are made for reports, letters, forms etc. which do not come under the NHS and are therefore chargeable.

A current list of charges is displayed in reception.

Please allow at least 10 days for all letters/forms/reports before collection.

Private Treatments

We can provide for private patients.

Please enquire at reception for consultation charges.

Complaints & Suggestions

We are committed to providing the best possible care.

If however, you have any suggestions for improving our service or have a complaint about any of our service you may ask to speak to the Practice Manager

Training

Churchill Medical Centre is a proud and active player in the training of tomorrow's GPs. We offer extensive training to both Registrars and Medical Students. Whenever possible we like to include them however, it cannot be stressed enough, it is you the patient who comes first and if you do not wish to partake in the training process then you have the right to say NO by simply informing your Doctor or the reception.

Registrars

A GP registrar is a qualified doctor with hospital experience who is completing their General Practice experience under supervision.

To further their skills and with your prior permission, some of their consultations will be video recorded as part of their training. We value and appreciate your cooperation in this matter, however you have the right to decline filming by simply informing the doctor or reception before hand.

Medical Students

This is a teaching practice for undergraduate teaching, therefore medical students will be undertaking some of their education in this practice. Consequently you may see a medical student in addition to your doctor.
This is an important part of their training as future doctors but please be assured that all student activity is closely supervised by your doctor.
Should you prefer to see the doctor alone without a student present then you are free to do so. This will not prejudice your treatment or care in any way.
Please inform reception if you would prefer to be seen without a student present.
